We are looking for a vibrant, bilingual, Medication Adherence Coordinator to work in a locally owned family practice environment at Southside and Medical Center based facilities. Certification as a Medical Assistant is Required. This is a team oriented, fast-paced, and patient centered practice.



Requirements:



Certified Medical Assistant with 2 years' experience with a strong preference for Family Practice. Candidates without 2 years' experience MAY be considered based on education, employment, and professional background.



Strong preference for bilingual candidates with EMR knowledge (eClinicalWorks). As a Medication Adherence Coordinator, under the direction of our provider(s), you will monitor patients' medication adherence daily, communicate with pharmacies, and call patients for reminders to pick up medication. Candidates will dispense medications with provider approval and in accordance with state and federal regulations. Candidates will interact with commercial, Medicare and Medicare Advantage plans and develop knowledge of chronic care management, HEDIS and Quality Metrics.
